70 yr old woman with right upper quadrant pain.

What does the radiograph show ?

Air in the wall of the gallbladder (emphysematous cholecystitis).

What is the usual cause ?

This is usually secondary to gallstones and infection.

What medical condition predisposes the patient to this ?

Diabetes Mellitus.
