What does the radiograph show ?

There is a fracture of the tibia and a lytic process affecting the tibia and fibula. This is a pathological fracture.

What process is likely to be happening at this site ?

This looks like an aggressive process and malignancy is more likely than infection

Where is the pathology based ?

Given the involvement of the fibula this is likely to be a soft tissue sarcoma rather than a bone tumour.
