What abnormalities are seen ?

Nodular shadowing is seen throughout both lungs and there are multiple pulmonary masses.

Are these changes likely to be new or old ?

These changes are likely to be old but previous films are needed for comparison.

Is previous occupational history
relevent ?

The paient was a coal miner. The appearance is consistent with pneumoconiosis. The larger pulmonary masses are due to Progressive Massive Fibrosis (PMF) - a complication of pneumoconiosis.
