An elderly patient with a history of falls is admitted with increasing confusion. .

What can you see on this non contrast CT?

There are bilateral fluid collections outside the brain. The shape of the fluid collections suggests that these are in the subdural space.

Are the abnormalities acute or chronic?

The fluid in the collections is generally the same density as the CSF so these are chronic subdurals but there are also areas of higher density seen within the collections. The areas of higher density are acute haemorrhage so these are acute on chronic subdural haematomas.

What medical co-morbidities may be present?

Many of these patients may be on anticoagulants such as warfarin. Other possibilities include impaired liver function in alcoholics who may also suffer from falls.

What treatment is indicated?

Urgent referral to a neurosurgical unit is advised to arrange drainage.
