History :
A middle aged woman became short of breath and was admitted to hospital.

Answers :

What can you see on the chest radiograph ?

The patient has a pneumothorax and an air fluid level - this is a hydropneumothorax.

Now look at the CT images - what underlying abnormality is present ?

The CT shows numerous bullae and cysts. The hydropneumothorax could be due to trauma but the presence of underlying lung disease raises the possibilty of a spontaneous pneumothorax.

On the CT there is another significant abnormality - what is it ?

There is left lower lobe collapse.
