History :
A young man presents with recurrent urinary tract infection.

Answers :

What does the radiograph show ?

Multiple small calculi are seen overlying both kidneys - nephrocalcinosis

What is the diagnosis ?

The appearance is suggestive of Medullary sponge kidney (the diagnosis can not be made by plain films alone)

In which structure, precisely, does the abnormality lie ?

In MSK multiple small cysts arise from the tubules in the medulla of the kidneys. The patient is susceptible to the formation of calculi.

What would you expect to see on an ultrasound scan ?

The ultrasound shows high echogenicity in the medullary pyramids consistent with calculi
