History :
A middle aged man has a routine chest radiograph.

Answers :

What does the radiograph show ?

There is an azygous fissure (seen in the right upper zone)

Do you think this patient has symptoms ?

This is a normal variant occurring in approximately 1% of the population. It is clinically insignificant and does not cause symptoms. It should not be confused with pathology.

The fissure is formed as the aygous vein passes through the lung taking with it four layers of pleura and so forms a 'fissure'. The azygous vein can be seen at the medial end of the fissure.
